Most recurring deficiencies on commercial fire doors fall into predictable categories. Knowing them in advance shortens inspection prep, reduces rework, and keeps your program moving forward.

Frequent Deficiencies

  • Improper clearances — undercut, top, or side gaps exceeding current NFPA 80 allowances
  • Latching failure — door does not positively latch under full self-closing force
  • Missing, damaged, or unreadable fire-rating labels on door or frame
  • Self-closing failure — door does not close fully from any open position without manual assist
  • Unapproved modifications — field cuts, added glass, non-listed hardware, or altered assemblies
  • Coordinator sequencing failure on pair openings — active leaf closing before inactive

Correction Strategy

  • Prioritize life-safety critical items first: latching failure, self-close failure, and blocked egress hardware
  • Group repairs by floor or zone to reduce disruption and minimize mobilization time
  • Submit out-of-scope materials — gap seals, hardware components — for written approval before purchase
  • Document each correction with technician, date, and method as part of the deficiency log
  • Re-inspect corrected openings and record closure before removing from the open deficiency list

Program Management

  • Establish a recurring annual inspection schedule — most AHJs require minimum annual review
  • Maintain a persistent opening-level deficiency log that tracks status across inspection cycles
  • Assign clear ownership for corrective actions — open items with no owner tend to stay open
  • Package summary reports for facilities leadership and AHJ records access
  • Track time-to-closure on deficiencies to identify systemic issues in specific zones or buildings
